feat: normalize database schema for document-chunk separation (Issue-2) ...

- Added 'documents' table for metadata.
- Refactored 'items' table for chunk storage.
- Updated MCP tools (add_item_text, search_text, etc.) to support the new schema.
- Updated technical documentation.
